I acknowledge that I am the legal parent/guardian of the minor(s) I have listed on this form. I understand the Grayson Bible Baptist Church is not liable for any harm or injury to my child/children that may result during activities hosted on the property of Grayson Bible Baptist Church. I give the adult leadership and workers of Grayson Bible Baptist Church authorization to assist any of my children in the case that he/she is injured or becomes sick. If there are any legal necessities that your child should not appear on any of our church's social media accounts, please contact the church office at (903)892-2141. I agree to abide by all policies and procedures of Grayson Bible Baptist Church concerning children's and youth ministry.
